Important Forms

This YBC Permission Form is needed to be filled out for every event that a YBC member comes to. This is so you/your child is covered by our insurance and by agreeing to follow the event rules, we can better manage the event. If the YBC member comes to the event but does not have the form , then they will not be allowed to attend the event. It is important that it is filled out and signed by you(if over 18)/your Parent/Guardian(if under 18).

This YBC Membership and Medical Release Form must be completed at the beginning of each year. It allows the YBC to update its membership records, and also if in an emergency have that YBC Member treated. It also allows the YBC to take photos of you/your child/ren at events throughout the year. There is no obligation for you to allow us to use photos, however, it would make the publicity of the YBC easier. Your cooperation is appreciated.
